Payment Options & Policies

Insurance may help pay for your weight loss visits!
- Commercial Insurance (Blue Cross/Blue Shield, Aetna, Ambetter, Cigna, Humana, UMR, United, etc.)
  - Resolve is an in-network participating provider with most insurance companies and we will gladly file a claim on your behalf for potential payment of fees paid for weight loss visits.
  - If your visits are covered by insurance, you are responsible for paying any deductibles or copays, and any additional services or products not covered by insurance.
  - If your visits are NOT covered by insurance, you will be charged our self-pay rates.
- Medicare/Tricare
  - Resolve is NOT currently accepting any new Medicare or Tricare patients at this time.
  - Resolve is a participating provider and will file your claims directly with Medicare and/or Tricare for current patients.
  - Medicare patients are responsible for paying deductibles and copays, and for services or products not covered by Medicare.
- Medicaid
  - Resolve is NOT a Medicaid provider and cannot file claims to any Medicaid plan.
  - Patients with Medicaid will be charged Resolve's self-pay fees.

No Show/Cancellation Fee is $60
- To ensure that each patient receives the care and attention they deserve, Resolve does not overbook/double-book our appointments. Accordingly, we ask that you provide us with at least 24 hours notice if you need to change your appointment. This will allow us the opportunity to offer the time slot to another patient who may be waiting for an appointment. If you don't provide 24 hours notice or miss your appointment without notifying Resolve you will be charged a $60 fee.
No Refunds
- Due to Legal and Medical restrictions Resolve is unable to offer refunds for any product (e.g., B-12 injections, supplements) purchased in our office.

Medical Visit Pricing (Self-Pay)

Initial Weight Loss Consultation ($235)

Your initial weight loss consultation will last approximately 45 minutes-1 hour and includes:

Extended visit with Resolve's Clinical Director, Jennifer Robinson, FNP-C to review and discuss your medical and weight loss history and to develop a personalized weight loss program just for you
Customized diet, nutrition, exercise and meal plan counseling
Prescription for weight loss medication, if health care provider determines it is medically appropriate.
Please note that the cost of any prescribed medication is not included in the price of the initial consultation. The cost of the medication is paid directly to the dispensing pharmacy.

Follow Up Weight Loss Consultation ($135)

Your follow up consultation lasts approximately 15-30 minutes and includes:

Additional one-on-one time with Resolve's Clinical Director, Jennifer Robinson, FNP-C. Jennifer will review your weight loss progress and adjust your personalized weight loss plan as needed to ensure that you are safely losing weight
Continued diet, nutrition, exercise and meal plan counseling to create lifestyle habits that result in long-term weight loss
Prescription for weight loss medication, if health care provider determines it is medically appropriate.
Please note that the cost of any prescribed medication is not included in the price of the follow up consultation. The cost of the medication is paid directly to the dispensing pharmacy.
